These are the necessary forms to be completed before the students first class. Download them here and bring them with you completed. All forms must be completely filled out with parental signatures and fees paid for students to participate.  

•The Medical Treatment Form is being requested because even though parents will be with their children, we need to have these authorization forms in cases of emergencies.
